Our experiences in pancreas and periampullary masses performed surgical intervention

Abstract (EN)

In our study, between 2001-2011, 79 patients having radical operation, had been made a retrospective evalution due to the mass in the pancreas and periampullary in the Department of General Surgery, Çukurova University Balcalı Hospital;In our seventy-nine patients, were 38 male (48,1 %) and 41female (51,9 %). The average age of male cases was 57 and the average age of female cases was found as 56.In between cases, the most common reason of complaints to our clinic had abdominal pain (74,6 %). Following this,31 cases with jaundice.Patients before surgery, were radiologically evaluated with US, BT and MR . In 32 cases (40,5 %) proved to have an Abdominal Ultrasonography were detected hydropic in the gallbladder, the expansion in choledochus and intra-hepatic biliary ducts, and also the pancreatic mass in 20 cases (25,3 %). 39 of patients( 90,6 %) that have a mass in their pancreas was able to see the mass with BT. In 16 of 18 patients (88,8 %) with MR also detected the lesion in the region of pancreatic and periampullary.A pylorus-preserving pancreaticoduodenectonmy was performed in 59 cases (74,7 %),a subtotal pancreatectomy splenectomy in 11 cases (14 %) and a distal pancreatectomy splenectomy in 9 cases.The average duration of surgery was found as 240 minutes (60-360 minutes)The blood transfusion was used on the average 2.8 units (0-8 units) during the operation. In peroperative four cases (5,1 %) had vascular injury. Primary vascular repair was preferred for these cases.In four each of patients with pylorus-preserving pancreaticoduodenectonmy developed biliary fistula (6,7 %) and pancreaticjejunostomy leakage (6,7 %). Developing fistulas closed spontaneously with the medical treatment in all casesIn two cases (5,8 %) developed ileus. It improved with the medical treatment. In twenty cases (2%) occured wound site infection. In four of patients (5,2 %) with pylorus-preserving pancreaticoduodenectonmy developed gastric atony. Any significant obstruction was not detected in grapy of contrast enchanced passage and improved with medical treatment.In our study, our 79 cases of pancreatic and periampullary cancer have been examined from various aspects and discussed under the literature researches.We are in the opinion of resection to the patients seen as resectable, using an adequate levels in the diagnosis and staging to the least invasive and most effective radiological methods for the patients whose general status is suitable for surgery.Keywords: Pancreas cancer, periampullary tumor, pylorus-preserving pancreaticoduodenectonmy
