Affect of clopidogrel receivement duration on inflamattory process that drug eluted and bare metal stent implanted patient.

Coronary artery disease is the most dominant disease because of its mortality and morbidity in this century. Although great developments in the field of PCI tecniques, re-stenosis and thrombosis are still important problems. Restenosis rates are not expected ratio in spite of decrease by drug eluting stents (DES). Dual anti-platelet (aspirin-clopidogrel) therapy lowers stent thrombosis risk clearly. It is known that periprocedurel imflammation causes adverse clinical outcomes in PCI. In this study, we aimed to investigate the effect of clopidogrel on inflammatory process during DES and bare metal stent (BMS) implantation and it?s relation with duration of clopidogrel therapy.Sixty patients with angiographically significant (>70 %) coronary lesions eligible for PCI were included to study. Patients were divided into 3 groups; BMS1group(n=20) contains patients with BMS and 1 month clopidogrel therapy, BMS6 (n=20) contains patients with BMS and 6 months clopidogrel therapy, and DES group (n=20) contains patients with DES (paclitaxel) and 6 months clopidogrel therapy. Biochemical, hematologic and inflammatory parameters were studied at baseline and inflammatory parameters were repeated at 1, 3 and 6 months of the study.Hs-CRP levels were significantly decreased in BMS1 group at 1 month (p<0.001) and hs-CRP levels were started to increase after cessation of the clopidogrel and returned to baseline levels at 6 months (p=0.174). The decrease in hs-CRP levels were observed at 1 month and reached statistically significant levels (p<0.001) at 3 months in BMS6 group and this significance was extended until 6 months (p<0.001). Hs-CRP levels were significantly decreased in DES GROUP at 1 month (p<0.001) and this significance was extended until 6 months (p<0.001). Any correlation between fibrinogen and VWF with hs-CRP were observed in any groups (p>0.05).In conclusion, clopidogrel lowers hs-CRP levels paralel to its administration duration.
