Abstract (EN)
Emergency departments are the units where patient flow is intense, fast and accurate. In recent years, increased emergency service intensity complicates patient management, decreases patient satisfaction and increases emergency service expenditures. Within this context, current triage systems can predict the urgency of patients but not predict hospitalization requirements. In this study, we evaluated two established clinical scores; GAPS and START in hospitalization predictions and their superiority in predicting hospitalization requirements during emergency service triage application. In this prospective observational study, all patients admitted to Akdeniz University Hospital Emergency Service Adult Triage Area between 01-08 June 2018 were evaluated. GAPS and START scores of patients were calculated during the triage and then patients were checked for their discharge from hospital or hospitalization. The ability of both scorings for predicting hospitalization and their comparisons with each other were analyzed with AUC values in the ROC analysis curve. A total of 2117 patients were enrolled in the study and 236 (11,1%) were hospitalized. The AUC value of the GAPS score was 0.894 (95% CI 0.881 to 0.907), the AUC value of the START score was 0.819 (95% CI 0.801 to 0.835) and both scoring predictions were high but the GAPS is a significantly better predictor of need for hospitalization than START (P <0.0001). In predicting hospitalization during triage in emergency services, both GAPS and START could be used with high hospitalization predictions while GAPS is a better predictor of need for hospital admission than START.
