The role of mr-proadrenomedullin in diagnosis and determining the severity of COVID-19 pneumonia

Abstract (EN)
Introduction-Aim: In this study, it was aimed to evaluate the contribution of MR proADM to diagnostic processes and its effectiveness in revealing the severity of the disease in Covid-19 patients. Materials and Methods: As a prospective, parallel group study, in Sakarya Training and Research Hospital Emergency Service, over 18 years of age, without severe immunodeficiency, without pulmonary abscess and aspiration pneumonia, not pregnant, no history of surgery in the last month, without a history of trauma and patients presenting with Covid-19 symptoms were divided into four groups according to RT-PCR and chest CT results. (Group-1: RT-PCR negative, chest CT negative (n=20), Group-2: RT-PCR positive, chest CT negative (n=20), Group-3 RT-PCR negative, chest CT positive (n=20), Group-4: RT-PCR positive, chest CT positive (n=28)) Demographic data, clinical features, vital signs, laboratory values, imaging methods, CURB65 Score, PSI Score and RT-PCR results of the patients were analyzed with IBM SPSS 21 software. Results: The mean age of the patients participating in the study was 53.30 (min:18, max:90) and 60.2% were women. When the complaints of the patients at the time of first admission to the hospital were evaluated, it was determined that malaise was the most common complaint with a rate of 58% and the most common comorbidity was HT (38.6%). Considering the RT-PCR result; although the WBC count was significantly lower in the group with positive RT-PCR results compared to the group with negative results, no significant results were observed in other laboratory values. According to chest CT results; while there was a significant decrease in WBC count in the group with positive chest CT, a statistically significant increase was found in CRP, Ferritin and D-Dimer results. RT-PCR positivity and chest CT positivity are statistically significant in terms of hospitalization, RT-PCR negativity and chest CT negativity are statistically significant in terms of discharge. There was no significant difference between the groups in Mr-proADM values, RT-PCR and chest CT findings. However, a significant difference was found between the high-risk group and the low-risk group in Mr-proADM values according to the PSI score (p:0.008). Conclusion: Although Mr-proADM levels are not suitable for the diagnostic approach in Covid-19, it can be used in the emergency room to evaluate the severity of the disease. However, additional studies with larger series are needed to support this data.
