Our hepatectomy results in giant primary liver tumors

Abstract (EN)

Primary Liver Tumors are the most common tumors in particular geography of eastern. For this reason, often focuses on the different treatment modellings. In our study was conducted the retrospective clinical analysis of huge primary liver tumors with hepatectomy in the General Surgery Department ,Faculty of Medicine, Cukurova University in between 1997-2012 years.In this study, the demographic information, histological tumour type, tumour size and tumour place, preoperative laboratory and radiological examinations, adjuvant methods of preoperative used, hepatectomy techniques, perioperative blood transfusion, operation time, postoperative hospital stay, postoperative mortality and morbidity, recurrence rates and survival were analyzed in 68 cases treated .45 cases (66.2%) were male and 23 cases were female. The average age was 50.2 (15-70). After histopathologic investigation of patients, hepatocellular carcinom 46 (67.8%), mesenchymal tumours 7 (10.3%), cholangiocellular carcinoma 6 (8.8%),neuroendocrine tumour 6 (8.8%), cyst adenocarcinoma 2 (2.2%) and non-Hodkign lymphoma 1 (2.2%) were found.Chronic liver disease were detected in 31 of 68 patients with hepatectomy, all of these cases were hepatocellular carsinoma.The different surgical techniques were performed to the cases in according to the location in liver of tumour, tumour size and general condition of patient. As a treatment, the right hepatectomy was performed for 38 patients (55.9%) , extended right hepatectomy for 7 (10.3%), left hepatectomy for 18 (26.5%), left lateral hepatectomy for 2 (2.9%), caudate lobectomy for 2 (2.9%), left hepatectomy and caudate lobectomy for 1 (1.5%).The average operation duration was 158.3 (s.s+-46.4) minutes. Transfusion used in the operation was 3.2 (s.s±3,1)units on average. Postoperative hospital stay time of cases was 14.4 (s.s±9,3) days on average.The complications developed in 26 cases (38.2%).While developing recurrence in 21 (30.9%) in the followup of patients, not detected recurrence in 47 (69.1%). 78.26% (45) cases were living at the end of 1st year, 58.93% (34) cases at the end of 2nd year and 46.22% (26) at the end of 5th year and 7.01% (4) at the end of 10th year.As a result of all these findings and the literature information examined ,We opinion that the major hepatic resections can be done with reduced morbidity and mortality rates as increasing the clinical experience, preoperative evaluation, radiological facilities and technological support .Keywords: Liver, Primary, Hepatocellular carcinoma, Hepatectomy, Complication
