Abstract (EN)
Diabetes; It is a metabolic disorder characterized by hyperglycemia, which can result in serious comorbidities and mortality, the incidence of which is increasing day by day worldwide. With the treatment modalities developed for patients with diabetes, the average life expectancy of patients with diabetes increases. This also increases the incidence of diabetes-related complications.The importance of preventing the development of these complications and regulating their treatment is increasing day by day. One of the most important complications due to diabetes is the diabetic foot. Diabetic foot; It is a complication of diabetes that can progress from diabetic foot sores at different levels in the foot to amputation as a result. Diabetic foot is the most common cause of nontraumatic lower extremity amputations. Satisfactory treatment methods have not yet been developed to determine the risk factors for diabetic foot diagnosis patients going to amputation and to prevent amputation. In this study, blood samples were taken from patients diagnosed with diabetic foot by applying to Gaziantep University Faculty of Medicine Orthopedics and Traumatology Clinic and Endocrinology and Metabolic Diseases Clinic. Thiol-disulfide, an important antioxidant, and 8-OHdG, an important indicator of oxidant mechanism, were studied from the blood samples taken from the patients. Patients who went to amputation as a result of an average of 9.6(6-16) months of follow-up were examined in terms of these parameters. Thiol-disulfide and 8-OHdG levels are measured to determine whether oxidative and antioxidative mechanism is a risk factor for amputation. In addition, it was investigated whether there is a relationship between the age, gender, Wagner level of the patients and amputation. As a result of our study, there was no significant relationship between amputation and 8-OHdG levels and thiyol-disulfide levels in diabetic foot diagnosis patients. The mean age of the patients who went to amputation and the Wagner level were found to be higher than those who did not. As a result, thiol-disulfide, which we used to evaluate antioxidant capacity, and 8-OHdG values, which we used to evaluate oxidant status, were not seen as risk factors for amputation. In patients with diabetic foot diagnosis, there was no correlation between oxidative stress and antioxidant capacity in the progression of amputation. Previously defined factors such as neuropathy, microvascular disorder, poor glycemic control, age, foot care seem to be more important in the progression of amputation. It is understood that it will be more beneficial to treat these factors, educate and raise awareness of patients in order to prevent amputation.
