Symptom burden, compliance with fluid restrictions, and affecting factors in diabetic and non-diabetic hemodialysis patients

This study was conducted to compare the level of compliance with symptom control and fluid restriction in diabetic and non-diabetic hemodialysis patients. The research was carried to out in four hemodialysis centers between between October 14, 2024, and December 10, 2024. The study was included 200 hemodialysis patients. The research data were collected "Patient Information Form" , "Dialysis Symptom Index (DSI)" and "Fluid Control Scale on Hemodialysis Patients (FCHPS)". In the data analysis was used Pearson Chi-Square Test, Yates Correction, Fisher's Exact Test with Monte Carlo Correction and Fisher's Excat Test in examining categorical variables according to groups, Bonferroni Corrected Z Test, Independent Samples t Test, Mann Whitney U Testi in pairwise comparisons. The DSI score of the diabetic group was higher than the non-diabetic group and a statistically significant difference was found between the groups in terms of symptom burden (p<0,05). The FCHPS score of the diabetic group was higher than the non-diabetic group and a statistically significant difference was found between the groups in terms of fluid restriction (p<0,05). A statistically significant relationship was found between the groups in terms of having problems during the HD session, session duration, dry weight, pre-dialysis, post-dialysis and interdialytic weight (p<0,05). In conclusion, in all individuals participating in the research was determined low symptom level and medium level of fluid restriction compliance. The symptom burden and fluid restriction compliance of the diabetic group was found to have higher than non-diabetic. It was found that the diabetic group's knowledge and behavior regarding fluid restriction were similar to those of non-diabetics, but their attitudes were lower. When providing care to individuals receiving hemodialysis treatment, nurses are recommended to apply more specific and broad-spectrum care to diabetic patients than to non-diabetic patients.
