The frequencies of asymptomatic bacteriuria and urinary tract infection in diabetic patients, the risk factors, and the association with procalcitonin levels

In this study, it was aimed to identify the frequencies of asymptomatic bacteriuria(ASB) and urinary tract infection (UTI) in diabetic patients, to uncover the risk factors,and to compare the ASB and UTI groups regarding the procalcitonin (PCT) levels.The laboratory tests that are required for Diabetes Mellitus (DM) were performedon 200 patients who were inpatients with the diagnosis of DM in the Internal MedicineClinic of our hospital. The patients who had symptoms suggestive of urinary tractinfection and positive urine culture were considered as symptomatic UTI. The patientswho did not have urinary tract infection complaints and had two positive urine culturesof the same type of bacteria with a concentration of 105 colony forming unit (cfu)/mLwere considered as ASB. The PCT levels were measured by Time Resolved AmplifiedCryptate Emission (TRACE) technique in the patients? serum samples. Statisticalanalysis of the data obtained in our study was made with SPSS software, Version10.0.Female gender, being equal to or over the age of 51, being widower and single,illiterateness, menopause, long duration of diabetes, presence of diabetic retinopathyand nephropathy complications, accompanying coronary artery disease,cerebrovascular event, and hypertension, presence of urinary catheterization, andhistory of UTI and any antibiotic use in last six months in diabetic patients weredetected as risk factors for UTI?s. It was revealed that PCT level was high in patientswith symptomatic UTI. In addition, C-reactive protein increase, leukocytosis, pyuria,fasting blood glucose, elevated blood urea, and creatinine were found associated withbacteriuria. The most frequently isolated bacteria in symptomatic UTI and ASB wasdetected to be Escherichia coli.In conclusion, the patients would be provided appropriate antibiotic treatment withearly diagnosis of UTI by knowing the frequencies of ASB and UTI, the risk factors,the most frequently isolated bacteria, and its sensitivity and measuring PCT level indiabetic patients. Therefore, the costs would decreased by shortened hospitalizationduration of the patient and the morbidity and mortality that could develop with UTIwould be prevented.
