Abstract (EN)
Secondary to the increase in active life and sports activities, most commonly injured joint is knee joint. The anterior and posterior cruciate ligaments, meniscal tears and cartilage lesions are the most frequent knee injury types. The aim of this study was to assess the diagnostic value of 3T MRI in demonstrating intra-articular pathologies of the knee such as meniscal structures and the structural elements of cartilage and ligaments. Between December 2005 - December 2010, 161 patients knee MRI findings were compared with knee arthroscopy which is the gold standard for evaluation. MRI findings sensitivity, specificity, positive and negative predictive values were calculated with the consistency rates. Intraobserver, interobserver kappa statistics were used in the comparison with arthroscopy. The sensitivity of MRI in detecting meniscal tears for observer 1a, 1b and 2, 91.3%, 79.6%, 89.8% ratios were found and the specificity ratios were 92.5%, 88.8% and 90.4%. The sensitivity of MRI to determine the full thickness ACL tear, observer 1a, 1b and 2, 99.3% 100% - 100% ratios were found and the specifity ratios were 82.6 %, 82.6% and %82.6. The evaluation of the kappa values for the presence of chondromalacia for observer 1a, 1b and 2 were 0,429, 0,488 and 0,489. The values were consistent with a moderate. In conclusion, MR imaging in the diagnosis of meniscal tear and ACL tears way found to be satisfactory in accordance with the literature, but were insufficient in the diagnosis of cartilage injury. The reason for this inadequacy as indicated in many studies is the limited diagnostic capability of MRI in the detection of the cartilage pathologies. On the other hand with the advent of more sophisticated MRI techniques which is capable to demonstrate cartilage physiology inadequacy of MRI in this era is getting improved.
