Evaluation of classical prognosis criteria in patients with primary membranous glomerulonephritis in nephrology clinic of Dokuz Eylul University Hospital

Abstract (EN)
Objectives: The aim of this study was to evaluate the classical prognosis criteria in patients with primary membranous glomerulonephritis. Method: Our research was designed as a retrospective study and was performed between November 1, 2017 and July 1, 2018. The research group; Patients aged 18 years or older who received biopsy examination and clinical evaluation of primary membranous nephropathy, who met the biopsy criteria (at least 8 non-sclerotic glomerular tissue) were included in our study. Demographic-clinical data of the study group, histopathological findings obtained from renal biopsy, and treatments of patients were recorded. Statistical Package for Social Sciences (SPSS) for Windows 22.0 package program was used for the statistical analysis. Findings: Among 54 patients with membranous nephropathy who were admitted to our clinic; A total of 24 patients were excluded from the study due to the fact that 3 patients were secondary MN, 12 patients had other center biopsy, 2 patients had inadequate glomerular count and 7 patients had inadequate laboratory data. The remaining 30 patients had evaluated with clinical and laboratory characteristics, general and renal survival through the treatment response. In parallel with the literature, the age, male gender, presence of hypertension, amount of proteinuria and high kidney chronicity at diagnosis score were observed to have a negative effect on disease prognosis. The fact that statistically significant p value has not been reached is thought to be due to the number of patients is not sufficient. In our study, cases with a daily urinary protein leakage of more than 8 grams and a kidney chronocity score of 2-3 were found to be difficult to respond to treatment, with fewer remissions and more frequent relapses. Result: In parallel with the literature, there was an unfavorable effect on the survival of patients with advanced age, male sex, presence of hypertension, weight of proteinuria (especially 8 gr/day) and presence of findings that advanced stage histopathologic damage. Close monitoring of proteinuria amount of patients will provide assessing risk class, remission and relapse status, and will be guidance in the choice of treatment.
