Analysis of preoperative and postoperative quality of life sexual function and sleep in patients with ebdometriosis

Introducion: Endometriosis is a chronic gynecological disease characterized by the presence of endometrial gland and tissue outside the uterus. It may progress asymptomatically, or may progress with symptoms such as infertility, chronic pelvic pain, dysmenorrhea, and dyspareunia. In addition to chronic physical pelvic pain, due to this pain, it also affects the quality of life, sleep quality and sexual life of the patients In this study, using validated questionnaires we compare the quality of life, sleep and sexual function of endometriosis patients before and after the operation. Our aim was to determine the benefit of operation for quality of life for endometriozis patients. Materials and Methods: 56 women aged between 18-60 years with endometriozis symptoms, applied to the Gynecology and Obstetrics Clinic at Bezmialem Vakıf University Faculty of Medicine Hospital between 2020-2021 were included the study. All of the patients have been operated with a preliminary diagnosis of endometriosis and the diagnosis of endometriosis was confirmed histopathologically. The preoperative and posteropative 3rd month quality scale questionnaires were applied to all of the patients. Evaluation with VAS for dysmenorrhea, dyspareunia and chronic pelvic pain before and 3 months after the operation and Pittsburgh Sleep Quality Index (PSOI), Morning-Evening Questionnaire (Turkish Version Of The MEQ), Endometriosis Health Profile Questionnaire (EHP- 30), the Female Sexual Function Scale (FSFI) Questionnaire was administered. These two periods were compared and analyzed. Results: Fifty six female patients included in the study The mean age of them were 36.82 ± 7.77 (22-54). The rates according to ASRM staging were as follows; 35.7% (n=20) Stage 1, 28.6% (n=16) Stage 2,14.3% (n=8) Stage 3, 21.4% (n=12) Stage 4. Statistically significant difference was observed in the pain scores, life, sexual function and sleep quality of the patients after the operation (p<0.001). In our study, we found that quality of life of the patients was improved. Conclusion: Endometriosis is a disease that negatively affects patients' quality of life, sexual function and sleep, especially due to high pain scores. Surgical or medical treatment can be applied for the complaints of the patients, considering their fertility status. We showed that the pain scores of the patients regressed and the quality of life, sexual function and sleep improved positively with the operation. We thought that operation is a good choice for endometriozis patients in order to improve the quality of their life.
