Abstract (EN)

This study was conducted as a non-randomized, control group experimental design to evaluate the effect of maternal voice and music intervention on pain, comfort and physiological parameters in children undergoing endotracheal aspiration procedure in pediatric intensive care unit. The population of the study consisted of children who underwent endotracheal aspiration at the pediatric intensive care clinic of the a training and research hospital affiliated with the Istanbul Provincial Health Directorate between September 2022 and June 2023, and 84 children (control group, control group) who met the criteria included in the treatment data at their centers and underwent endotracheal aspiration. n) =30; music group, n=30; mother voice group, n=24) was created."Child Descriptive Information Form" in which information about the child was obtained, "Physiologic Parameters Follow-up Form" in which physiologic parameters (heart rate, respiratory rate, oxygen saturation, systolic and diastolic blood pressure) of the experimental and control groups pre, during and post-procedure were recorded, "Comfort Scale" for distress/comfort and sedation assessment, "FLACC Pain Scale" for the assessment of pain level were utilized for data collection. In order to determine the homogeneity of the groups for quantitative variables, the chi-square test was conducted for categorical variables and the anova test was conducted for continuous variables. Intergroup comparisons were evaluated by one-way analysis of variance. The post-hoc test was utilized for multiple comparisons, and repeated measures analysis of variance and mixed anova were employed in the evaluation of repeated measures. Data were evaluated at p<0.05 significance level. It was observed that the groups were similar in terms of gender, age, duration of hospitalization, duration of ventilator exposure, aspiration frequency, aspiration pressure, saline use, and sedation level of the children included in the study (p>0.05). Although FLACC pain scale mean scores were higher during the procedure compared to other procedure times for each group, they were determined to be lower in the music intervention and maternal voice group compared to the control group (p<0.001). Although there was a statistically significant increase in comfort scores during the procedure (p<0.001), there was no significant difference between the groups in terms of mean comfort scores (p>0.05). In terms of physiological parameters, while there was no significant difference between the groups (p>0.05), heart rate, systolic and diastolic blood pressure increased in the control group during the procedure (p<0.001), respiratory rate decreased in the music intervention group during the procedure (p<0.05), and oxygen saturation after the procedure was higher in the maternal voice group compared to the control group in the intergroup evaluation (p<0.05). These results indicate that maternal voice and music intervention are efficacious in managing pain during the endotracheal aspiration procedure, and it is recommended that the intervention of maternal voice and music should be increased in pediatric intensive care units to eliminate the traumatic effects of pain.
