Research on the effect of amnion membrane application on mortality in adult burn

Abstract (EN)
MATERIAL AND METHOD: 494 pateints who had treatment between 2013 and 2016 were enrolled to the study. Patients in all groups were evaluated statistically by age, sex, percentage of burns, type of burn, duration of hospital stay, culture results, number of surgical procedures, burn complications and mortality. Logistic regression was used for determination of risk factors that effect of mortality. FINDING: %40,9 of the study population had the application of amniotic membrane whereas %59,1 of them did not receive the application. 164 female and 333 male patients were admitted to the study. %57,5(284) of the population had full-thickness burns and %42,5 (240) had partial thickness burns. Logistic regression model showed that usage of amniotic membrane in burn management decreased the mortality and provided twofold protectivity ( odds ratio:0.467 confidence interval: %95 P=0,0001). When the cut-off level determined as %30 of total burned surface area, mortality increases 5,35 fold in patients with higher TBSA. ABSI is a risk factor. Every one score increase in ABSI increases the mortality 1,99 fold. Wound culture positivity is also a risk factor. Every positivity in wound culture increases the mortality 2,68 fold. Development of any complication in burn management is also another risk factor. Sepsis and renal failure increases motality 2,83 and 5,8 fold respectively. (P=0,0001) RESULTS: İn Conclusion application of amniotic membrane in severely burned patients (with high burn severity index) decreases burn mortality.
