Stage-4 in patients with colon cancerprognostic value of neutrophil/ albumin ratio

Abstract (EN)
INTRODUCTION AND OBJECTİVES: Colon cancer is a severe health disease that can be seen sporadic or related with hereditary risk factors. All over the world; after the prostat cancer in men and breast cancer in women, colon cancer is second most prevalant cancer. The aim of this study was to determine whether neutrophil/albumin ratios at the time of diagnosis of colorectal cancer are associated with prognosis. MATERIALS AND METHODS: This study included 140 stage-IV colon cancer patients older than 18 years who were diagnosed with colorectal cancer between March 2006 and June 2018. The cases were reviewed retrospectively from the aspect of demographic datas such as age and gender; tumor locations, metastasis sites, chemotherapy regimens and surveys. Neutrophil, lymphocyte, platelet and albumin levels at the time of diagnosis were obtained from venous blood serum. FINDINGS: Significant [area under the curve 0.645 (0.543-0.748)] efficacy of the NAR value was observed in differentiating the proggressive cases from the nonprogressive ones, according to NAR values. At NAR -2.2 cut-off value, regarding to differentiate the cases that were getting progressed from the non-progressive ones; sensitivity, positive predictive value, specificity and negative predictive value were 60.7%, 75.0%, 64.7% and 48.5%, respectively. According to the expected survival; 95% of confidence interval predicted survival time of 16.8-23.2 (mean 20.0) months in the NAR >2.2 group was significantly (p < 0.05) lower than the predicted survival time of 24.6-33.2 (mean 28.9) months in the NAR <2.2 group. CONCLUSION AND RESULTS: In this study, in stage-IV colon cancer patients, cases with high NAR values were associated with poor prognosis and short survey. However, we think that more multicentric studies with larger patient groups and more homogeneous distribution of the sample, except for the NAR value, are needed to make a clear conclusion that the NAR value is a prognostic biomarker in stage-4 colon cancer patients.
