The effect of delivery unit satisfaction on perceptions of traumatic birth in women giving birth in different centers

This study was conducted to determine the effect of the birth unit satisfaction of women who gave vaginal birth in different centers on their perception of traumatic birth. The study, planned in a descriptive design, was conducted with 275 puerperal women who had vaginal birth in two hospitals in Tokat province. Data were collected using the "Sociodemographic Data Form", "Bırth Unıt Satısfactıon Evaluatıon Scale" and "Traumatic Birth Perception Scale (TBPS)" developed in line with the literature. The obtained data were analyzed in SPSS 22.0 program. In the evaluation of the data, t-test in independent groups, one-way ANOVA and Tukey HSD and Tamhane's tests from Post-Hoc tests were used. The relationship between the data was evaluated with Pearson Correlation analysis; the predictive level was determined with simple linear regression analysis. In statistical tests, the significance level was accepted as 95% confidence interval(p<0.05). The average age of women giving birth was found to be 27.58±5.10 years; the average duration of marriage was found to be 5.65±4.57 years. The total average score of the women on the Bırth Unıt Satısfactıon Evaluatıon Scalewas found to be 122.49±11.35, and the total average score of the Traumatic Birth Perception Scale was found to be 68.47±25.01. A statistically significant difference was found between the total average scores of the Bırth Unıt Satısfactıon Evaluatıon Scaleand the Traumatic Birth Perception Scale according to the center where the women gave birth (p<0.05). It was observed that the mean scores of the Bırth Unıt Satısfactıon Evaluatıon Scale sub-dimensions Communication and Care, Comfort and Birth Support Facilities of women giving birth in university hospitals were higher than those of women giving birth in state hospitals; the mean scores of the Traumatic Birth Perception Scale were lower. It was determined that the satisfaction with the physical characteristics of the delivery room was lower in those living in the city center and in older pregnant women; the comfort satisfaction was lower in women whose spouses were unemployed and primary school graduates, and the communication and care satisfaction was higher in those living in a nuclear family. In addition, it was determined that as the gestational week increased, the communication and care, physical characteristics of the delivery room and comfort satisfaction increased (p<0.05). The mean score of the Traumatic Birth Perception Scale did not show a significant difference according to sociodemographic and obstetric characteristics (p>0.05). When the relationship between the scales was examined; a statistically significant and negative relationship was found between the Bırth Unıt Satısfactıon Evaluatıon Scaleand the Traumatic Birth Perception Scale (F=115.021; p<0.05). According to the regression analysis, %29.6 of the traumatic birth perception is explained by the birth unit satisfaction level. A 1-unit increase in birth unit satisfaction caused a 1.319-unit decrease in the perception of traumatic birth (p<0.05). According to the findings of the study; it was seen that the delivery unit satisfaction of the women was high, their perception of traumatic birth was at a moderate level; as the delivery unit satisfaction level increased, the perception of traumatic birth decreased and the delivery unit satisfaction levels of the women who gave birth in university hospitals were higher than those who gave birth in state hospitals and their perception of traumatic birth was low. In this respect, increasing the quality of delivery environments will contribute to women having a more positive delivery experience. In this respect, it is recommended that delivery units be designed in line with the opinions of the women who gave birth and the midwives and considering the suitability criteria.
