Abstract (EN)

Efficiency of Magnetic Resonance Imaging (MRI) according to FIGO 2009 endometrium cancer stage AİM: In this study, we aimed to evaluate the efficacy levels in detecting the depth of myometrial invasion and cervical stromal involvement by comparing the results of preoperative magnetic resonance imaging (MRI) and intraoperative frozen examination with the final pathology reports to evaluate myometrial invasion, cervical stromal involvement and adnexal spread. MATERIALS AND METHODS: In this retrospective study, 156 patients diagnosed with endometrial cancer as a result of probe curettage performed for diagnostic purposes in Sakarya University Sakarya Training and Research Hospital Gynecology and Obstetrics Clinic between January 2015 and December 2021 were evaluated with MRI for depth of myometrial invasion and cervical stromal involvement, and frozen during the operation. examination results were evaluated. The results of both methods were compared with the final pathology reports, which is considered the gold standard. The data were analyzed using the SPSS 20.0 statistical package program. RESULTS: 156 patients were included in the study. The ages of the patients were between 37 and 84 years, and the mean age was 61,09±8,753 years. In the evaluation of the depth of myometrial invasion; The sensitivity, specificity, PPV, NPV, and overall accuracy of MRI were 66,1%, 95%, 88,1%, 83,3%, and 84,6%, respectively, while the values of frozen examination were 74,5%, 98,9%, 90%, respectively. It was found as 82,4% and 90,4%. In the evaluation of cervical stromal involvement; while the sensitivity, specificity, PPV, NPV and overall accuracy values of MRI were 60% 97,1% 69,2% 95,8% and 93,5%, respectively, the values of frozen examination were 66,6% 99,2% 88%, respectively was found to be 97% and 96,5%. In addition, factors such as histological grade, tumor size, LVSI and cervical stromal involvement were found to be effective in the depth of myometrial invasion. Myometrial invasion was found to be a prognostic factor in pelvic and paraaortic lymph node involvement. CONCLUSION: Preoperative MRI seems to be an important method in determining the depth of myometrial invasion and cervical stromal involvement in endometrial cancer due to its high accuracy rates.
