The role of the aortic cava index in predicting hypotension after general anesthesia induction in gastrointestinal tumor surgery

The purpose of our study: The purpose of our study conducted at Dicle University Faculty of Medicine is to investigate the predictability of intraoperative hypotension developing after general anesthesia by the cava aortic index in patients undergoing surgery for gastrointestinal tumors. Method: Our current study was conducted by obtaining permission from the Ethics Committee of Non-Interventional Studies of Dicle University Faculty of Medicine dated 12.06.2024 with the ethics number 332. The study in question was conducted by including 32 patients aged 18-75, ASA 1-2-3, who underwent emergency and elective surgery in the operating room of Dicle University Faculty of Medicine. The exclusion criteria are as follows; patients not wanting to be included in our study, pregnant patients, obese patients with BMI>30, patients whose aorta and inferior vena cava could not be visualized with USG, patients diagnosed with hypertension and patients with high intra-abdominal pressure. Results: Of the 32 patients included in this study, 15 patients who developed hypotension were defined as group 1 and 17 patients who did not develop hypotension were defined as group 2. According to the results we obtained in the demographic data of the patients, there was no statistically significant difference. We also did not detect any statistically significant difference between the groups in terms of the cava aortic index and inferior vena cava collapsibility index. Conclusion of our study: According to the results we obtained, the cava aortic index calculated by measuring the preoperative vena cava inferior and abdominal aorta diameters in patients who will undergo surgery under general anesthesia due to gastrointestinal tumor surgery cannot predict the hypotension that develops in patients. We think that the cava aortic index should be applicable to all patient groups in order to become a standard in terms of predicting hypotension.
