Abstract (EN)

Advances in antiretroviral therapy (ART) have brought the life expectancy of individuals living with HIV infection (HIID) closer to the general population. Antiretroviral treatment cause persistent immunological activation, chronic inflammation, and a number of comorbidities to arise more frequently and at younger ages than in the general population. ART have a favorable effect on mortality and morbidity in HIV-affected people. The metabolic syndrome, osteoporosis, osteopenia, osteomalacia, dyslipidemia, diabetes, and heart disorders are among the comorbidities that typically accompany with HIID. The objective of this study was to assess the metabolic parameters (liver functions, kidney functions, skeletal system), and to compare changes in blood lipids, fasting blood sugar, and kidney function tests with the antiretroviral medications they use in patients who applied to Gaziantep University Faculty of Medicine Infectious Diseases and Clinical Microbiology outpatient clinics. Demographic data and habitual traits such as smoking and alcohol use of 247 patients included in the study were examined. The patients in our study ranged in age from 18 to 69 years old, with a mean age of 37.49 (±12.4). The patients were mostly male (86.6%) and heterosexual (63.4%). Individuals with HIV had a significantly greater risk of psychiatric problems as compared to the control group. While the DEXA score indicated osteopenia in the majority of the patients, a significant portion of them also had vitamin D deficiency. While DTG+TDF/FTC was the most assigned initial ART regimen with 23.1%, there was a statistically significant increase in total cholesterol and LDL cholesterol levels at the end of the first year in patients whose treatment strategy included TAF. The goal was to identify the metabolic parameters that should be continuously monitored during HIV disease and to enable medication customisation for particular patient groups. With the increase in life expectancy in HIID, a multidisciplinary strategy is necessary to effectively manage the comorbidities and metabolic side effects that rise with age.
