Abstract (EN)

Dr. Kıvılcım Ertürk. A Search About Myocardial Protection And Effects On Myocardial Functions By Using Cardioplegia With Or Without Glutamate Aspartate. Medical Speciality Thesis, Bolu, 2010. It?s recommended that ingredients of cardioplegic solutions and practise methods need to be developed for stonger myocardial protection. In this aim, insulin, L carnitine, cortisol, ß blockers, Ca channel blockers, NAD, glutamate aspartate and simillar substances added in to cardioplegic solutions and performed via coronary arteries (antegrad), coronary sinus (retrograd) or simultaneously. Practising local or systemic hypotermia contributed to be success about myocardial protection. With this methods, myocardial damage was decreased but could not be terminated.In this study it?s analysed two group of patient performed cardioplegy with or without Glutamate Aspartate in Coronary Artery Bypass Grafting operation. Patients in both two groups evaluated with cardiac damage markers that routinly studies in our clinic; CK, CK-MB, Trophonine I, LDH, AST, C Reactive Protein (CRP), ProBNP at first hour, first day, second day and fifth day after operation. In adition cardiac output (CO) measured and cardiac index (CI) calculated at preoperative, peroperative (after decanulation ), postoperative first hour, first day, second day and compared effects of using cardioplegy with or without Glutamate Aspartate on myocardial performance was compared.In conclusion, it?s observed that 14 patient performed cardioplegy with Glutamate Aspartate exhibited better cardiac performance, needed lesser positive inotropic support than the patients of other group. And there is statistical significant diffrence between two group about pre and postoperational CO, CI, and ejection fraction. Although there is no statistical significant diffrence between 2 groups in cardiac damage markers, high levels was measured in control group (performed cardioplegy without Glutamate Aspartate) .Key Words: Glutamate, aspartate, cardioplegia, cardiopulmonary by-pass grafting, myocardial protection
