Utility of mean platelet volume (MPV) for diagnosing acute coronary syndrome in patients with chest pain

Abstract (EN)
Chest pain is one of the serious reasons need to be interest and evaluate that most of them life threatening at adults emergency departments. The life threatening reasons are mostly about cardiac events. Acute coronary syndromes are the most serious reasons that cause chest pain among the cardiac etiologies. ACS is a challenging situation to evaulate according the type of chest pain and subgroup of acute coronary syndrome. Mostly, it needs to get serial ECG and serial measurement of cardiac enzyme level that the duration of evaluation the patient takes at least 24 hours and sometimes the diagnosis can be missed out. At this point, it needs to be neccesary to research some new biomarkers for realizing ACS earlier. Our study was planned with the idea that mean platelet volume (MPV) which is useful to determine the functions of platelets can contribute the course diagnosing ACS. This retrospective study included 681 patients that was admitted to Adıyaman University Research and Training Hospital Emergency Department with chest pain between 01.01.2017- 31.10.2018. ACS diagnosis was clarified according to the results of percutaneous coronary intervention (PCI). Patients with coronary obstruction> %70 was diagnosed as ACS and put in Group-1 (86%), coronary obstruction <%70 did not have ACS diagnose and put in Group-2 (14%). MPV values were investigated between groups and we found that MPV parameter had an increasing pattern at AKS patients (mean value of MPV in Group-1= 7,92 fL; mean value of MPV in Group-2= 7,65 fL; p<0,01). MPV's dignosing performance for ACS was analysed with ROC curve. We determined cut-off value as 7,40 fL with sensitivity %50 and specificity %65. We also found inreased mean MPV value in patients with ischemic findings in ECG are positive in Group-1 (p<0,01). There is not an relationship between cTnI and MPV statistically (p=0,417). Consequently, MPV is one of the biomarkers that can be used to diagnose ACS. There are some diffuculties at standardization of MPV for clinical usage. It needs to get some comprehensive researchs to bring in this biomarker that is easy to measure, to use in clinical cases. Key Words: MPV, acute coronary syndrome, troponin I, chest pain, ECG
