A comparison of effect of intraartricular application of hyaluronic acid and tenoxicam in term of knee joint function and pain in patients with grade 2-3 gonarthrosis

Abstract (EN)
OA is a non inflammatory chronic degenerative disease characterized by progressive destruction of cartilage, osteophyte formation and subchondral sclerosis especially on load bearing joints. So many patients consult a doctor every year because of joint pain, decrease of ROM and consequently loss of function. İn our study we aimed to compare symptomatic therapeutic efficacy of intraarticular tenoxicam and intraarticular HA application on patients with a diagnosis of Grade II-III gonarthrosis. We chose 60 patients for our study with a diagnosis of clinically Grade II-III gonarthrosis to our study, who came to AIBU medical faculty , Orthopedics and Traumatology clinic with knee pain. patients were divided into two groups consist of 30 patients. For 3 weeks long we have aplied intraarticuler HA 1 dose per week to the patients in Group I (n:30). We applied only 1 dose intraarticuler tenoxicam to patients in Group II (n:30).We examined patients before treatment and 1.-15. days and 1. , 2. and 3. months after treatment. We evaluated ROM and VAS for therapeutic efficacy . To analize the data we used SPSS 15.0 statistical analysis program. To compare pre and post-treatment data, we used wilcoxon test thecnique. And also we used mann whitney u test to compare flexion and extansion datas and VAS datas of patients who had used different drugs in different periods. In 2 months period after treatment we found statistically significant difference between VAS datas of Group I and Group II (p<0.05). VAS datas were higher in Group I in first 2 months period after treatment. 3 months after treatment there wasnt any statistically significant difference between vas datas of GroupI and II.(p>0.05). In first 15 day period after treatment we found statistically significant difference between rom datas of Group I and Group II (p<0.05).rom datas were higher in Group II . 15 days after treatment there wasnt any statistically significant difference between rom datas of Group I and II.(p>0.05). In our study patient satisfaction was higher in Group II in early period of study and it was higher in Group I in late period of study when compared with pre-treatment period. In early period tenoxicam group was superior about patient satisfaction, but 3 months after treatment there was no difference between two groups. As a result in early period after injection when compared with HA , Tenoxicam can provide better rom and pain scores. We think that intraarticuler Tenoxicam injection is a safe, low cost, fast-acting, good tolerable symptomatic treatment alternative which reduces NSAI usage and their complications.
