Abstract (EN)

Our study?s aim is to compare security, facility of practise and early complications of the Griggs, Percu Twist and Ciaglia tracheotomy techniques? which are the methods of percutaneous tracheotomy.After the acknowledgment of Faculty Ethic Committee, patients were implicated to the study after ratification of themselves or kins. Patients were randomly seperated to the 3 groups of 20 patients. Groups were named Group G for the group using Griggs method, Group P for PercuTwist and Group C for Ciaglia. Dilatation couldn?t be occured in 3 patients of Group P and passed to another method. The patients were applied 2 mg kg?¹ propofol, 1µg kg?¹ fentanyl and 0.1 mg kg?¹ vecuronium with sedation and analgesia. Patient?s head and neck were got extantion by putting a roll under their shoulders.Period, procedure of dilatation, tracheotomy canula replacement, bleeding during the process and after the process, blood suctioned from lungs after the process, time of staying in ICU, saturation of peripheral oxigen, experience of doctor, value of Hb1 before process and Hb2 after 24 hours of process and early complications in first 24 hours were compared.A meaningful statistical difference couldn?t be found between patient?s demografic datas, time of staying in ICU, Hb1 and Hb2, experience of doctor, results of recovery and exitus. The shortest process time occured in Group G (191.55±110.6 second). The procedure of dilatation, the replacement of tracheotomy canula was the easiest in Group G and the most difficult in Group P. During and after the process more bleeding ascertained in Group C than Groups G and P. Blood aspired from lungs after the process was found the least in Group P and the most in Groups C and G. Decrease of SpO2 was seen in Groups P (6), C (7) and in Group G (3 patients) (p<0.05). In Griggs method cardiac arrest was noticed in a patient. A major complication wasn?t ascertained in all groups.In conclusion, we can use all of the methods of percutaneous trachetomy with broncoscopy in a short time and confidence. Especially we recommed the methods of Griggs and Percu Twist that has started to be used recently.
