Özet (EN)

The purpose of the study to describe of the medication errors related to the use of the medications which patients use before they are taken to the hospital and after they are admitted to the clinic and the relation of these medication errors in the nursing form and nursing history. 156 surgical patient were reviwed. Data were collected with a questionnaire and a nurse registration form. The 48% of the patients have continued to use the medication that they used before they are admitted to the hospital. It has been found that in nursing history, the 74.4% of the patients and, in the nursing forms, the 66.0% of the patients use error of medication before they are admitted to the hospital. In the nursing history, it has been observed that there is an error of dose and frequency in the 39.7% of patients. In nursing forms, there has been a medication omission error in the 47,6% of the patients. It has been determined that, though they continue using their medication in the service, the 12% of the patients aren?t registered in nursing history, and the 38.7% of them in nursing forms. The high error ratio in the registration forms has shown that the nurses haven?t taken efficient and right history. That the nurses and doctors aren?t in the know about the medications which the patients continue using in the service may cause mediciation interactions. It has been suggested that there should be researches into the improvement form of the medication reconciliation ve into the analysis of the reasons giving rise to the formation of errors and differences.
