Retrospective analysis of target HbA1c level attainment rates and influencing factors in type 2 diabetic patients with HbA1c levels above 10% AIM: Diabetes mellitus (DM) is a multisystemic chronic disease characterized by elevated blood glucose levels due to complete or partial absence of insulin and ineffective use of insulin secondary to tissue resistance to insulin. In this study, we planned to retrospectively examine the achievement of target HbA1c levels, achievement and the factors affecting the achievement of these targets in type 2 diabetic patients with HbA1c levels above 10. MATERIALS AND METHODS: In this study, the medical records of 466 type 2 diabetes patients with HbA1c level above 10 who were admitted to the diabetes outpatient clinic of Sakarya University Hospital, Department of Internal Medicine between January 1, 2022 and December 31, 2023, and who had at 3- or 6-month intervals were retrospectively analyzed for achievement of target HbA1c level and glycemic regulation rates. Laboratory parameters were measured. It was planned to compare the laboratory parameters of insulin-treated patients and patients who did not accept insulin treatment and received only oral antidiabetic treatment at the time of initial presentation and at follow-up visits. RESULTS: Our study included 466 patients with HbA1c ≥ 10 who presented to the diabetes outpatient clinic. Of the patients, 274 were using insulin and 192 were using OADs. Of the participants, 273 were female and 193 were male. HbA1c at admission was 11.5±1.3 in OAD users and 8.3±2.1 in the control group. In insulin users, HbA1c was 12±1.7 at admission and 9±2 in the control group. CONCLUSION: In our study, the rates of HbA1c reduction and achievement of glycemic regulation were similar between OAD users and insulin-treated patients. The rate of reaching target HbA1c (HbA1c ≤7) was higher in OAD users.
