Comparision of magnetic resonance cholangiopancreatography and endoscopic retrograde cholangiopancreatography in hepatobiliary disorders

Magnetic resonance cholangiopancreatography, is a non-invasive technique which enables high-resolution imaging of the bile duct and pancreatic duct. The studies compared with ERCP for diagnosis of hepatobiliary pathology has revealed high sensitivity and specificity. The diagnostic field of this activity varies depending on the type of disease. In our study, we planned to evaluate the effectiveness of MRCP for diagnosis, comparing it with ERCP in patients admitted to our department. Files of patients who underwent MRCP and ERCP, were evaluated retrospectively. A total of 110 patients ( 53 men, 57 women) were included in the study. The mean age was 63.56 ± 16.897. When ERCP was accepted as the gold standard method; and patients, who were unsuccessful for biliary cannulation were excluded; sensitivity and specificity of MRCP for choledocholithiasis were (66.66%,82.85%) respectively. The positive and negative predictive value of MRCP for choledocholithiasis were (86.95%,59.18%) respectively. The sensitivity of MRCP for stones less than 6mm was 35%, when the patients in geriatric age group is removed, sensitivity of MRCP for choledocholithiasis was (92.3%). When ERCP was accepted as the gold standard method, MRCP 's sensitivity and specificity for hepatobiliary malignancies were (69.56%,100%), while positive and negative predictive values for hepatobiliary malignancies were (100%, 91.13%), respectively. MRCP shows high sensitivity and specificity values in the diagnosis of hepatobiliary disease. Hence, it is preferably used in diagnostic field instead of ERCP. Along with the development of technology, the efficiency of MRCP in the field of diagnosis is expected to grow
