Comparison of nebivolol and quinapril affects on the endothelium disfunction in hipertensive patients

Abstract (EN)

2. ABSTRACTIf endothelial disfunction can be detected and treated at early stage,atherosclerotic involvoment can be prevented. The aim of this study is to comparethe improvement in endothelial dysfunction at early stage under the quinapril ornebivolol treatment.Sixty hypertensive patients at stage I and II according to JNC 7 criteria aretaken in to this study. This patient were categorized in to two groups; quinapril(n=30) and nebivolol (n=30), first group was medicated with quinapril 20 mg/dayand the second group with nebivolol 5 mg/day for four weeks. The FMD (Flowmediated dilatation) methods are used in this study. Also, resistive index, pulsatil Tindex, intima media thickness, flow volume, V max states were analyzed and werecompared with each other.Two groups were similar about age, gender, left ventricule mass index, leftventricule ejection fractions and body mass index. No significant differences werefound regarding atherosclerotic risk factors and other echocardiographic,demographic and biochemical measures between groups. After four weekssignificant reduction in SKB and DKB levels was recognized in both quinapril andnebivolol groups. In the quinapril group, FMD increased after 4 weeks of treatmentperiod but this did not reach at statistical level (% 4,77±3,92; %5,60±6,18p=0,587). However in nebivolol group the difference in FMD levels before and aftertreatment period (%3,78±4,25 ; %8,56±6,39) was statistically significant (p= 0,002).When we compared the groups about the effects of medications and endothelialinduced vazodilatation, we found that nebivolol group showed a near-significantdifference (p=0,064).The aim of the modern antihypertensive medication is to regulate bloodpressure and beyond to prevent target tissue damage leading mortality and morbidity.We suggest that the effects of using antihypertensives which have useful and earlyeffect on endothelial disfunction such as nebivolol increase in future several years.Keywords: Endothelial dysfunction, Hypertension, Quinapril, Nebivolol2
