Abstract (EN)

INTRODUCTION AND AIM: Patients living with HIV (PLWH) have got different comorbidities becoming more frequent with the increase of life expectancy. It is important to compare information from real-life cases, as different treatments have disadvantages and advantages. In this study, it was aimed to determine real-life data regarding the efficacy, safety, and evaluation of clinical and laboratory findings in the course of three different antiretroviral treatments in CAD. METHODS: In this study, 54 retrospective patients file data contained with Dolutegravir & Lamivudine (3TC/DTG), Tenofovir Alafenamide & Emtricitabine & Elvitegravir & Cobicistat (EVG/c/TAF/FTC) and Tenofovir Disoproxil Fumarate & Emtricitabine & Efavirenz (EFV/FTC/TDF) made using the data. Data were observed at baseline and at 12 months after antiretroviral therapy (ART), and the all data were statistically compared. RESULTS: Although a total of 54 PLWH were included in the study but, 7 PLWH (six in the EFV/FTC/TDF group, one in the EVG/c/TAF/FTC group) were excluded from the study The study was completed with 47 patients. Of the patients, 22 (46.8%) were in the 3TC/DTG group, 19 (40.4%) were in the EVG/c/TAF/FTC group, and 6 (12.8%) were in the EFV/FTC/TDF group. After one year therapy with 3TC/DTG; BMI, HIV RNA, CD4, WBC, hemoglobin, MCV, PDW, RDW, platelet count, creatinine, eGFR, HDL, AST, glucose, Beck depression scale, left femur and spine thickness values change were statistically significant (p<0.05). After 12 months of treatment, BMI, HIV RNA, CD4 count, MCV, creatinine, eGFR, HDL, LDL, TG, TK, AST, HOMA-IR, serum phosphorus, Beck depression scale were statistically significant in the EVG/c/TAF/FTC treatment group. difference was detected (p<0.05). After 12 months of treatment, there was a statistically significant difference in HIV RNA, total bilirubin, LDL, left femur thickness values in the EFV/FTC/TDF treatment group (p<0.05). CONCLUSION: All treatment groups had a decrease in HIV RNA and an increase in CD4 at the end of one year treatment. CD4 elevation in EFV recipients is less than in INSTI recipients. Weight gain is greater in those taking INSTI. In the group using EVG/c/TAF/FTC, the negative lipid profile and increase in insulin resistance were remarkable, and the lipid profile course was found to be safer in the 3TC/DTG group. The decrease in bilirubin in EFV/FTC/TDF patients was significantly different from other treatment groups.
