Prognostic factors in hormone receptor positive, HER2 negati̇ve operated breast cancer patients

Abstract (EN)

INTRODUCTION-AIM:Breast cancer is the most common and the most common cause of death among cancers. In developing countries, the risk of breast cancer is high. In recent years, early diagnosis and treatment progressed in breast cancer with screening programs and mortality rates decreased. Prognostic and predictive factors lead the physician to the appropriate treatment plan. Prognostic factors give information about the clinical course of the patient regardless of treatment; predictive factors include survival-related factors associated with survival. Molecular prognostic factors are tumor size, axillary lymph node metastasis, histopathological tumor type, grade, hormone receptors, tumor proliferation rate. In this study, we aimed to investigate the factors affecting survival in patients with hormone receptor positive and HER2-negative breast cancer. Materials and Methods: We retrospectively evaluated the hormone positive, HER2 negative and operated breast cancer patients who applied to the medical oncology clinic between January 1, 2012 and December 31, 2018. Patients under the age of 18, who received neoadjuvant therapy, those with metastatic stage, hormone negative and HER2 positive were not evaluated. In addition, patients who could not access the file information were excluded from the study.Results: In this period, we found 165 breast cancer patients who operated HER2 negative hormone positive during this period. The median age of the patients was 55 years (minimun 22-maximum 85) years. 61 (37%) of the patients were premenopausal and 104 (63%) were postmenopausal. Of the patients, 124 (75.2%) were invasive ductal carcinoma, 21 (12.7%) were invasive lobular carcinoma, 8 (4.8%) were papillary carcinoma, and 6 (3.6%) were mucinous carcinoma. 2 (1.2%) had tubular carcinoma pathology. The number of lymph nodes (p = 0.003), pathological stage of the tumor (p = 0.004) and TNM (p = 0.004) showed statistically significant differences in the number of survivors and survivors. The rate of Ki 67 ≥ 20% of the patients with recurrence was significantly higher than the patients without relapse(p=0.002). Conclusion: The prognostic factors of all breast cancer patients should be determined from the time of diagnosis and the treatment plan should be performed accordingly. Axillary lymph node involvement status and number, size of the tumor, estrogen-progesterone receptor status and HER2 status are the safe markers that can be used to determine the prognosis. Similar to other studies in our study, tumor size increase, increase in the number of lymph node involvement and Ki-67 percentage was found to be adversely affecting survival due to the increase in percentage.Key words: Hormone positivity, HER2 negativity, breast cancer, survival Key words: Hormone positivity, HER2 negativity, breast cancer, survival
