ABSTRACTDifferences in the mammographic findings of premenopausal and postmenopausalwomen taking hormonal therapyHormonal therapy has taken permission for three main endications in the wholeworld. These are the treatment of the vasomotor symptoms, the prevention of theurogenital atrophy and the prevention of the osteoporosis. For all these reasons, the benefitof the hormonal therapy which is regulated for the patient is certain. Although, thediscussions about it to incrase the breast canser stil continues.Mammography is a gold standart procedure in the early diagnosis of the breastcanser. It deareases the mortality dependent on the breast cancer in the 50 years old orolder women. However mammography is the most sensitive procedure in the diagnosis ofthe breast cancer, its sensitivity is less than 85-90%. The main reason for this is, thetendency of the dense breast tissue in hiding the lesion lying underneath. The interpretationof the mammographies is highly correspondent to the high risk of the breast cancer. Indifferent studies, hormonal therapy is related to the increase in the density of the breast in25% of the overall cases. This condition is important in the follow up the patients takinghormone therapy.Over goal in this study is to observe the changes in the mammogaphic findings ofthe women taking hormone therapy in the premenopausal and postmenopausal periods.Due to this aim, 630 cases, attending to the menopause clins of the gynecology andaobstetrics department of the Medicine Faculty of the Çukurova University betwen the yearJanuary 2000 and December 2005, are taken into the study. 441 patients were takinghormone therapy and 189 patients weren?t taking any therapy and this is control group.The mammographies of the patients before and after the hormone therapy were discussedretrospectively according to the changes in the pattern, lesion and density.At the end of the study tibolon and conjugated oral estrogen plus+MPA takinggroup shoud a 3% increase in the liposclerosan or sclerozan (dens) breast pattern, there isshowed is a 4% increase among cyclic therapy taking patients.Only oral estrogen taking group didn?t show any change in the pattern. Conversely,there was an increase in the lipomatous breast pattern among no therapy taking group andviiitransdermal estrogen taking group. The ratio of the dense breast pattern increased from 6%to 10% until the first mammography to the second mammography in HRT taking patients.The increase in the incidense of the lesions was 3.9% in the oral estrogen taking groupwhich was the highest increase. The increase in the density was 16% in the hormonetherapy taking group while it was 4.7% in no therapy taking group. The highest increase inthe density was 41% which was seen in the cyclic therapy taking group, it was 23% in theconjugated plus+ MPA in the conjugated estogen plus MPA taking group.Key words: menopause, hormonal therapy, mammogaphy.ix
