Evaluation of the effect of gonadotropin releasing hormone agonist therapy on body mass index in idiopathi̇c central precocious puberty

Abstract (EN)
Background:There are concerns that GnRHa may cause increase body mass index (BMI SDS) in patients with Central Precocious Puberty(CPP). In studies between GnRHa treatment and ΔBMISDS, conflicting results have been obtained. In this study, it was aimed to evaluate the effect of GnRHa treatment on BMI SDS. Materials and methods:Medical files of patients who were followed-up with the idiopathic CPP and given GnRHa treatment at least 2 years were evaluated retrospectively. The anthropometric data before treatment, at the 1st and 2nd years of treatment, at the end of treatment and in the 1st year after the treatment were recorded. The patients were divided into 3 groups as normal weight (NW), overweight (OW) and obese(OB) according to BMI SDS values before the treatment. It was also divided into 2 groups according to type of GnRHa used and they were younger or older than 8 years at the beginning of the treatment. Results:At the beginning, 58% of the cases were normal weight, 32% were overweight and 10% were obese. In the whole group the mean BMI SDS increased significantly in the first year of treatment compared to the baseline. Although it was still higher in the 2nd year of treatment, there was no increase compared to the 1st year. During follow-up, BMI SDS decreased and returned to baseline at the end of treatment.The NW group showed a significant increase in BMI SDS in the 1st year, while the OW/OB group did not show a significant increase.In patients <8 years of age, the BMI SDS was similar compared to baseline, in patients >8 years of age whereas the mean BMISDS increased significantly in the first year, compared to the baseline. Although it was still higher in the 2nd year, there was no increase compared to the 1st year. During follow-up, BMI SDS decreased and returned to baseline at the end of treatment.Although there was an insignificant increase in the BMI SDS in the first year in Leuprolide patients, in Triptorelin patients, the BMI SDS was significantly higher in the 1st and 2nd years but returned to baseline at the end of treatment. Conclusions:In our result GnRHa treatment caused a temporary increase in BMI SDS during treatment and returned baseline at the end of treatment. The increase in BMI SDS was related to the baseline BMI SDS, the age of treatment and whichGnRHa used. Keywords: GnRH agonist; central precocious puberty; obesity; body mass index (BMI)
