Abstract (EN)
Introduction and Goals: Burns have both physically and psychological trauma on human being with mortality and morbidity. It still gets to be a problem for physicians in acute and long term treatment for burns. Botulinum toxin has been used for; hiperkinetic and dystonic disorders, spasticity, facial asymetry, raynaud's phenomenon, migraine treatment, postoperative pectoral muscle spasm, delay for flap pedicles and also hpretrophic scar tissue treatment. As we examine the effects of Botulinum toxin A on the inflammatory phase, there might be an additionary effect on inflammation of stasis zone after burn. The aim of this study is the investigation of effects of Botulinum toxin A on stasis zone wound healing after burn trauma. Material and Methods: 20 adult Sprague-Dawley rats with a wight of 200-225 grams are used for this study, which were seperated into two, as 10 for control group and 10 for botox group. 90 mg/kg Ketamin-HCl (Ketalar®, Pfizer) and 10 mg/kg Xylazin-HCl (Rompun®, Bayer) are intramuscularly applied for anesthesia before the burn procedure. Second degree deep burn wounds are created at the back of the rats with a metal block kept waiting in a hot boiling water. After burn procedure; serum physiologic were applie to control group and 9 IU botulinum toxin A for each rat in the botox group. After seven days, marked radioactive material (Technetium 99 methoxyisobutylisonitrile) were injected through the tail veins which was followed by excision of the burned back wounds in order to be scanned in scintigraphy. Later on, these specimens were fixated with formaldehyde for histopatologic study. Hematoxylin eosin and Masson trichrom dying methods were applied to evaluate fibrosis levels, granulation, inflammatory response, vascularization, epitelization in the stasis zone. Symptoms: Scintigraphy scans have showed the tissue viability in the stasis zone to be dramaticly higher. Also, epitelization in the botox group were found to be significantly higher than the control group. Inflammation and granulation tissue were identified lower in the botox group. No significant difference in fibrosis and vascularization were found between two groups. Results: Under controlling of inflammatory response in the burn stasis wound healing is effective for reducing the damage of the trauma. Setting with this foresight, botulium toxin A might be effective in repressing the early inflammatory phase to reduce the reversible damage at the stasis zone. In our study, stasis zone with an applyment of botulinum toxin A was evaluated to have an antiinflammatuary effect. We thought botulinum toxin A in order to be used in other extended studies, to show the damage reduce in burns. Our study shows that immunohistochemical examination may be needed in order to show the molecular effects of Botulinum Toxin A. Keywords: Zone of stasis , Botulinum toxin A, Inflammation.
