Eskisehir Osmangazi University Faculty of Medicine Department of Internal Medicine, Eskisehir, 2025. Bladder cancer is the 2nd most common malignant tumour of the urinary system and ranks 10th among all cancers worldwide. Our study aims to examine the efficacy and side effects of gemcitabine-cisplatin and gemcitabine-carboplatin in the first-line treatment of inop/metastatic bladder cancer and to reach real-life data. A total of 79 patients who applied to Eskişehir Osmangazi University Medical Faculty Medical Oncology outpatient clinic between 2010 and 2023 and underwent gemcitabine-cisplatin and gemcitabine-carboplatin treatment in the first-line treatment of inop/metastatic bladder cancer were included in the study. Gender, age, performance score, lymphovascular invasion (LVI), neural invasion (NI), presence of metastasis, concomitant chronic diseases, haematological and non-haematological side effects after gemcitabine-cisplatin and gemcitabine-carboplatin treatment, progression-free and overall survival times, and mortality status were investigated based on pathology reports and imaging results. Seven (8.9%) of the patients were female and 72 (91.1%) were male. Gemcitabine-cisplatin protocol was applied to 34 (43%) and gemcitabine-carboplatin protocol to 45 (57%) patients. OS was 17.341±2.150 months for gemcitabine-cisplatin and 19.467±5.613 months for gemcitabine-carboplatin p=0.090; PS was 3.265±0.299 months for gemcitabine-cisplatin and 3.067±0.292 months for gemcitabine-carboplatin p=0.851. Haematological and non-haematological side effects were detected in all patients. In our study, in univariate analysis, ECOG performance score of 0-1, the number of CT cycles received ≥6, partial and stable response to CT, absence of liver metastasis, and fewer metastasis sites were found to be associated with longer OS. In univariate analysis, higher number of CT cycles and absence of LVI were found to be associated with longer OS. In multivariate analysis, a statistically significant relationship was found between the absence of liver metastasis and OS, and between the number of CT cycles and PS.
