Serum copeptin levels in the diagnosis and prognosis of cerebrovaskuler diseases (cvd)

Abstract (EN)
Cerebrovascular disease is among the most common mortis cause in our country just as it is in the whole world. The early diagnosis of cerebrovascular disease in important in that it enables the early detection of complications and early treatment. Copeptin is a part of pre-pro-vasopressin and is synthesized in equal molar amounts with vasopressin. It has been demonstrated that it reflects the endogenous stress level better than the cortisol. In cardiovascular diseases, sepsis, shock, paralysis, trauma and cancer, copeptin increases in the plasma in a fast pace. This quick increase is valuable in diagnosis and prognosis determination. In our study, in patients presenting to the emergency department with CVD, copeptin level was to evaluate the relationship with diagnosis and prognosis. Patients over the age of 18 who applied to the Emergency Service between the dates of October 2012-September 2013 with ischemic, hemorrhagic, and cerebrovascular diagnoses were included in the study. 150 patients participated in the study and 50 of them had cerebrovascular disease, 50 of them had hemorrhagic disease, 50 of them had traumatic diseases. 50 healthy people were also included in the study as the control group. Blood samples were collected from the patients during their applications to the hospital; and from the healthy control group during their applications for the study. Copeptin levels were found as 7,56,3 ng\dl (p<0,05) in patient diagnosed as ischemic CVD, 6,85,3 ng\dl (p<0,05) in patient diagnosed as hemorrhagic CVD, 7,24,7 ng\dl (p<0,05) in patient diagnosed as travmatic CVD, 2,11,0 ng\dl in control group. SVH serum copeptin levels of each group were compared with a control group with no significant difference was found (p <0.01). Serum copeptin levels and in-hospital prognosis of patients we look at the relationship between mortality and copeptin levels and a direct correlation between poor prognosis and increased mortality was observed. As a result, the group increased in SVH copeptin level, poor prognosis and is associated with increased mortality. This copeptin of us, prognosis and mortality can be used as markers for the calculation shows. Key Words: Cerebrovascular disease, Copeptin
