Abstract (EN)

Aim: The aim of this study is to compare socio-demographic characteristics, radiological features, treatment responses and drug resistance patterns of Beijing genotype TB patients with non-Beijing genotype patients in Istanbul Materials and Methods: Data were collected by retrospectively scanning the records of the cases registered to Tuberculosis Dispensaries in Istanbul between 2013-2019, whose drug resistance tests were performed, and whose genotype was determined by spoligotyping typing method in the Tuberculosis and Molecular Epidemiology unit of the Experimental Medicine Research Institute (DETAE) of İstanbul University. The files of a total of 232 patients with 86 Beijing strains and randomly selected 146 patients without Beijing strains were scanned and compared. Patients' age, gender, nationality, hometown, marital status, BCG scar, history of contact with a tuberculosis patient, presence of chronic disease, history of substance abuse, presence of HIV infection, smoking history, alcohol history, case definition, radiological patterns, presence of cavity, history of travel abroad, history of contact with a person from China/Eurasia region, presence of H/R/E/SM/H+R resistance, resistance to any drug, type of diagnosis, organ involved, treatment given, treatment results recorded in the patient registration form.. Findings: A total of 232 patients, 86 (37.1%) in the Beijing group and 146 (62.9%) in the control group, were included in the study. The age of the patients was significantly lower in the Beijing group compared to the control group (p<0.001); foreign nationality (p<0.001), travel abroad history (p<0.001), history of contact with a person from China/Eurasia region (p<0.001), rate of being single (p=0.004), extrapulmonary involvement (p=0.017), previous treatment history (p=0.003), treatment failure (p=0.001), H/R/E/SM/H+R resistance/resistance to any drug (p<0.001) were found to be significantly higher. When the R, H+R and resistance patterns of the new cases in the Beijing group and the control group were examined; the resistance pattern of the new cases in the Beijing group was significantly higher (p<0.001) than the new cases in the control group. When the group with R resistance and the group without R resistance are compared among the total cases; there was no significant difference in resistance patterns between Turkish nationals and foreign nationals in the Beijing group. Result: The Beijing strain, which has an increasing incidence in Turkey, is associated with drug resistance and treatment failure. It is thought that it may be useful to carry out further studies on whether routine genotyping will contribute to the control program in terms of providing effective tuberculosis control.
