Evaluation of perinatal outcomes of İVF pregnancies

Introduction: We aimed to examine the complications that may occur during the pregnancy period in IVF pregnancies and their effects on maternal and fetal mortality and morbidity with a retrospective study method and to determine the most appropriate ways in the management of these pregnancies. Materials and Methods: In this retrospective study, it was planned to retrospectively examine the files of 241 patients who gave birth in our hospital between January 1, 2015 and December 31, 2019 and who became pregnant with in vitro fertilization (IVF). In the analyzes the age, parity (nulliparous - multiparous) of the patients, smoking, development of gestational diabetes, development of gestational hypertension, development of preeclampsia or eclampsia, development of placenta previa, birth week, type of delivery (normal vaginal birth - cesarean section) was evaluated. At the same time in the study; It was planned to evaluate the pregnancy outcomes in IVF pregnancies by examining the baby's birth weight, the baby's 5th minute APGAR (activity, pulse, grimace, appearance and respiration) score, and fetal anomaly development. Benefits: In our study; we wanted to draw attention to the importance of having prior knowledge about the complications that may develop in the follow-up of IVF pregnancies, both the timely intervention by increasing the awareness of the obstetrician who will follow the pregnancy and the importance of informing the pregnant about these conditions. Perinatal results to be obtained from our study; it will guide obstetricians in matters such as the follow-up of pregnancy, the management of complications and the method of delivery. At the same time, with the multiple parameters examined, maternal and fetal mortality and morbidity will be minimized and the optimal management method will be determined. Results: A total of 241 patients who got pregnant with IVF and gave birth in our hospital were included in the study and were examined one by one according to the parameters. In our study, it was observed that the mean age of the mothers was 33,3 (Median: 33, min: 19, max: 58) years, while the mean birth weeks of the babies were 35.54 (median: 37, min: 23, max: 41) weeks, the birth weight of the babies and their average was determined to be 2480.55 (median: 2655, min: 200, max: 4320) kilos. When the pregnancies in the study were examined, it was found that 62.7% (n = 151) were singleton pregnancies, 33.2% (n = 80) were twin pregnancies and 4.1% (n = 10) were triplet pregnancies. While the presence of DM (diabetes mellitus) was not observed in 207 (85.9%) of the patients included in the study, the presence of PGDM (pregestational DM) in 4 (1.7%) patients and GDM (gestational DM) in 30 (12.4%) patients was detected. When the findings of the presence of HT (hypertension) are examined; While no signs of HT (hypertension) were found in 218 (90.5%) patients, 9 (3.7%) patients had chronic HT (chronic HT) and 14 (5.8%) patients had GHT (gestational HT) findings. When the delivery methods of the patients are examined; While 60 (24.9%) patients performed NVD (normal vaginal delivery), 181 (75.1%) patients delivered by C / S (cesarean section). Conclusions: The complication rates of IVF pregnancies are higher than spontaneous pregnancies, and their management also varies. It was observed that the IVF pregnancy outcomes of our clinic were similar to the results of other clinics where studies with a high number of patients were conducted. Healthy IVF pregnancies and healthy IVF generations can be achieved with new researches and sensitivities in pregnancy follow-up. Keywords: IVF (in vitro fertilization), perinatal, pregnancy outcomes
