Abstract (EN)
INTRODUCTION AND AIM: Introduction: C-section is one of the most common surgical procedures and is done to save the lives of the mother and baby in cases of complicated labour and pregnancy. Since World Health Organization (WHO) in 1985 declared that the C-section should be between 10-15 percent of all the deliveries, the number of operations have raised uncontrollably. High C-section delivery percentage doesn't only cause short-term perinatal and maternal complications due to unindicated operations but also long-term complications, public health problems and raise in the health expenses. This study is focusing on what the obstetricians think about the causes of raise in C-section numbers and their ideas on the possible solutions. MATERIALS AND METHODS: This study uses the quantitative research method and quantitative research pattern phenomenologically and is done in Sakarya which is one of the 30 big cities in Turkey and which is situated in the Marmara Region between 01.02.2020 and 01.09.2020. The people to be interviewed in this study has been chosen by the purposeful sampling, one of the improbable sampling methods . All the obstetricians interviewed in the study have completed a obstetrix and gynecology program (at least 4 year) and worked a minimum of 3 years as a doctor afterwards. The study collects data via deep interviewing method. We have taken the consent from the attendants to record the interviews and for those who doesn't consent to recording have we taken notes. In the meetings with the attendants we have used a questionairre that was made of two parts. The first part consisted of six questions related with the sociodemographical data while the second part consisted of half-constructed questions depending on the literature. The recorded interviews have been written down by the xvii researcher by the help of Microsoft Word. For the analysis of the written data have we used the thematic content analysis and reached subthemes and themes. RESULTS: 17 obstetricians have been deeply interviewed in this study. 12 male (70.6%) and 5 female (29.4%) were interviewed. The age distribution of the attendants were between 42-68 with standard deviation 59.64±6.7. 8(47.1%) of the attendants declared their monthly income as good and 9(52.9%) as normal. After the thematical analysis of the data collected by the interviews with these doctors, we created 60 categories, 16 subthemes and 4 main themes. Themes were: Possible reasons of high C-section rates, doctors' the suggestions for the solution of high C-section rates, possible outcomes of the C-sections, and being a specialist in the obstetrics and gynaecology field in Turkey. CONCLUSION: Because the rate of C-sections raise continously worldwide and in Turkey, we tried to find out what obstetricians think on the reasons of this and their suggestions to solve this situation. According to attendants, the reasons for high rate of C-sections are problems in the health system and politics, the justice system, and the doctor-patient relations play the biggest roles. Suggestions of the doctors were making corrections in the health and justice systems, solving problems in the midwifery system and supporting the doctors. The reasons contributing to the hig rate of C-sections are multifactorial and related to each other so the measures to be taken should also be comprehensive. We should specially focus on the operations which raise the rate of C-sections but medical indications are weak.
