BACKGROUND: High rates of morbidity and mortality are reported following hip fractures. Evaluating treatment methods in femoral neck fractures to determine the most effective treatment paradigm will become increasingly important. Most of the fracture are treated by arthroplasty or internal plate. The arthroplasty treatment and the internal plate have many failure. The dynamic or sliding hip screw (DHS) is the most commonly used implant for the hip fracture. In most of the case DHS are preffered for treatment of the femoral neck fracture but treatment of femoral neck fractures are still associated with a higher risk of postoperative complications. The dynamic hip screw device has several modes of failure, the most common of which is cut out of screws. The single screw in the femoral neck, which provides the main mode of fracture fixation, may be insufficient in providing rotational stability. PURPOSE: We try to develope the DHS and we added a ring gage on the femoral screw. The current study compared new ring gage added compression plate (RGCP) with dynamic sliding hip screw (DHS). METHODS: We compared the mechanical behaviour of osteosynthesis with the new ring added compression plate (RGCP) compared with the standard osteosynthesis dynamic sliding hip screw (DHS) in femoral neck hip fractures. We fixed the DHS with 7 synthetic femurs without any osteotomy in first group, osteotomy of the DHS with 7 synthetic femoral neck with DHS in second group and osteotomy of the DHS with 7 synthetic femoral neck with RGCP in third group. Each fracture was fixed with either the RGCP or DHS. All femoral heads were load until 2000 Newton by the compression-distraction machine (Shimatsu-Japon) and we used the video extensiometer to record the deplacement of the fracture fragment. RESULTS: The RACH group, less deplaced than the DHS group. These results showed that RACH (3.group ),which have been ring added DHS are more stabil sytem than the DHS group. CONCLUSIONS: Clinical prospective studies are needed to confirm these in vitro findings that the RGCP is more stable than the DHS.
