Factors affecting mortality in geriatric patients operated for hip fracture

Abstract (EN)

INTRODUCTION AND AİM: High mortality rates after hip fracture surgery is a major health problem. Many factors have been reported that affect postoperative mortality. In our study, we aimed to evaluate the factors affecting mortality in the postoperative 30-day period. MATERIAL AND METHODS: Our study included 906 patients aged 65 and over who had hip fracture surgery between 01/01/2015 and 01/06/2021 in our hospital. Patients were divided into two groups, those who died within 30 days and those who survived. Demographic data of patients, ASA classifications, comorbidities, anesthesia method, blood product transfusion, length of stay in hospital and intensive care units, preoperative and postoperative hemoglobin, platelet, INR, urea, creatinine, AST, ALT values on postoperative 30-day mortality were analyzed retrospectively. RESULTS: The postoperative 30-day mortality of the patients was found to be 8,6%. The mean age of the deceased was 83,49 ± 6,9, while the mean age of the survivors was 78,7 ± 7,6. Of the patients, 63,9% were female and 36,1% were male. Advanced age (OD: 1,091; CI 95%, 1,051-1,132), high ASA value (OD: 12,69; CI 95%, 1,074-150,17), coronary artery disease (OD: 0,521; CI 95%, 0,287-0,944), general anesthesia application (OD: 0,305; CI 95%, 0,140-0,667), high creatinine values (OD: 1,045; CI 95%, 1,114-1,892) were found to increase mortality. CONCLUSIONS: In our study, it was concluded that high age, ASA and creatinine values, presence of coronary artery disease, and application of general anesthesia increased mortality. We think that a detailed examination of elderly hip fracture patients in terms of these risk factors and perioperative close follow-up will reduce mortality.
