Evaluation of complications of central venous catheter mounted internal juguler vein in heart and vasculer surgery patient

Objective: Central venous catheterization is an important approach for patient follow up and fluid, blood and blood products replacement in anesthesia management of patients who have undergone open heart surgery. However, as with any invasive procedure, there are complications that may occur during central venous catheterization. It is important to know the complications that may occur in order to decrease the complication rate and the factors that are effective on complication development. This study aimed to determine the complications of central venous catheterization and the risk factors that could lead to the development of complications. Materials and Methods: This retrospective study was carried out by examining the data of the patients who underwent open heart surgery between October 2015 and March 2017 at Dokuz Eylül University Medical Faculty Hospital after obtaining permission from Ethics Committee for Noninvasive Research of Dokuz Eylul University Faculty of Medicine. Demographic data of the patients, characteristics of the operation they underwent, poncture related complications during catheterization, guidewire related complications and catheter related complications were examined. Results: Of the 404 patients examined in the study, 82 (20.3%) developed complications. The complications were detected as; hematoma development (12.1%), arterial puncture (6.9%), pneumothorax (0.24%), catheter obstruction (4%), catheter malposition (3.2%) and catheter infection (%2,2). Body mass index, number of percutaneus puncture, the side of catheterization,thyroid disease and hypertension, chemotherapy and radiotherapy history were found to be effective factors in the development of complications. Discussion and Conclusion: Although the patients who underwent open heart surgery had a high risk, heparinized, postoperative long term need for mechanical ventilation compared to the general population, the complication rates in our study were found to be similar to data in the literature. It has been determined that well-monitored patients and compliance with a certain form for optimal depth may be due to complication rate reduction.
