The effect of sevoflurane and desflurane on clara cell protein on the lung in liver transplant donors

Abstract (EN)
Aim: Inhalation anesthesia is the most common application method of general anesthesia. Inhalation anesthetics diffuse from the lung alveoli into the blood and reach the brain tissue, resulting in unconsciousness and amnesia. It is known that inhalation anesthetics produce different effects on the respiratory system except the anesthetic effect. Clara cells in the respiratory epithelium; it is known that they function in the detoxification of xenobiotic and oxidant gases, in the control of the area of inflammation, in the mucociliary clearance of environmental agents, proliferation and differentiation of ciliated cells. In this study, it was aimed to investigate the effects of sevoflurane and desflurane inhalation anesthetics on CC16 in liver transplant donors scheduled for hepatectomy surgery. Material and Method: A total of 78 patients aged between 18-65 years, who were donors of liver transplantation ASA I-II, who would undergo hepatectomy surgery were included in the study. The cases were divided into three groups as control, sevoflurane and desflurane groups. In the maintenance of anesthesia, the control group; propofol, remifentanil, O2/air, and other groups were used remifentanil, O2/air in addition to sevoflurane or desflurane. In order to evaluate CC16, blood samples were taken from all patients preoperatively (t1), at the first hour of the intraoperative period (t2) and at the end of the surgical procedure, after stopping the anesthetic drugs (t3). CC16 levels were analyzed by ELISA method. Results: There is a significant difference between the groups in terms of t1 CC16 values. The t1 CC16 values of the sevoflurane group were significantly lower than the control group. There was a significant difference between the groups in terms of t2 CC16 values. When the sevoflurane group was compared with the control group; t2 CC16 values were found to be significantly lower. The t3 CC16 levels were significantly different between the groups. t3 CC16 values were significantly lower in the sevoflurane group compared to the control group. In all groups, t3 CC16 levels were significantly lower than t1 CC16 levels. Conclusion: Sevoflurane and desflurane did not have a negative effect on serum CC16 levels. Sevoflurane was found to significantly lower the CC16 level when compared to desflurane. Key words: Clara cell protein, desflurane, hepatectomy, sevoflurane
