Comparison of combined and staged surgery procedures applied in combination of carotis and coronary artery disease

INTRODUCTION: In our study, we have discussed the mid-term results of the concomitant and stage surgery procedures which applied in the combination of carotid and coronary artery disease. MATERIALS AND METHODS: The study included 37 patients, who were diagnosed and operated with carotid and coronary artery disease at the Department of Cardiovascular Surgery, at Vakif University Hospital between January 2012 and February 2016. There were 22 patients (N1) concomitant (CEA + CABG) and 15 patients (N2) stage (first CEA followed by CABG) . The patients who have stabile preoperative hemodynamic,no unstable angina,no emergent CABG were operated with stage procedure.The perioperative morbidity and mortality of the patients were evaluated with medium term follow-upresults. DATA ANALYSIS: There was no statistically significant difference in preoperative demography data between the two groups (p>0.05). There was no early mortality. There was no statistically significant difference between the two groups in minor neurological(p>0.05). There was no major strocke,pertoperative MI or enfection of surgery incisions. Restenosis was not detected in the two groups at 6 months postoperatively. Carotid restenosis was detected one patient in each group with 50-69% rate at 12 months postoperatively. RESULT: According to the clinical priorities of the patient carotid endarterectomy with using carotid shunt,as stage or before sternotomy in combine procedure can be safely performed.
