Abstract (EN)

Both World Health Organization and United Nations declared chronic diseases as the most important health issue for the world in the 21st century. Burden of chronic diseases is also important in Turkey and threatens the sustainability of the healthcare system. Integration of healthcare systems is necessary to overcome of the burden of chronic diseases. Patient-centered healthcare models must be used. A paradigm shift from the traditional hospital-centered and reactive healthcare delivery model toward a person-centered and preventive one is required.In this study, in order to provide the management of chronic diseases with person-centered and preventive approach, attention is drawn to Personal Health Systems that aim to take healthcare services out of the hospital and bring them into the lives of individuals and contain wearable, pluggable and portable systems and technologies which are qualified new generation e-Health systems. Patient empowerment and personal health information management is highlighted throughout the study; as an application a portable, intelligent and adaptable personal health assistant is designed that help people with diabetes better manage their condition.
