Investigation of antibiotic resistance profile and virulance factors of Klebsiella pneumoniae isolates from clinical samples

Abstract (EN)
Klebsiella pneumoniae (K. pneumoniae) is a Gram-negative opportunistic pathogen that plays a role in hospital-acquired infections. Infections caused by K. pneumoniae are difficult to treat and have a high mortality because of the presence of beta-lactamase enzyme, resistance to antibiotics and biofilm formation. In the light of this information, in our study, the resistance profiles of K. pneumoniae (N=120) isolates obtained from outpatients and inpatients who applied to Tokat State Hospital, Tokat Erbaa State Hospital and Samsun Bafra State Hospital after obtaining ethics committee approval, showed that resistance profiles against various antibiotics were found to be associated with disc diffusion, phenotypic ESBL formation. It was aimed to determine the ESBL genes (blaTEM, blaSHV and blaCTX-M), type 1 adhesin (fimH) virulence factor, the presence of Class 1 integron (intI1) by PCR method, and biofilm formations by microtitration method. The resistance rates of clinical isolates according to the Kirby-Bauer disc diffusion method according to antibiotics; Of 120 isolates, 47 (39.1%) were treated with meropenem, 43 (35.8%) with ımipenem, 72 (60%) with ceftazidime, 48 (40%) with chloramphenicol, 107 (89.1%) to levofloxacin, 69 (57.5) to aztreonam, 71 (59.1%) to fosfomycin, 53 (44.1%) to tobramycin , 47 (39.1%) to gentamicin, 83 (69.1) to amoxacillin clavunata, 65 (54.1) to piperacillin tazobactam, 68 (56.6%) to It is resistant to nalidixic acid. According to the combined disk method to detect ESBL formation phenotypically; Of 120 isolates, 28 (23.3%) were ESBL positive. Biofilm formation results according to the microtitration plate method to detect biofilm formation; 58 (48.3%) of 120 isolates were biofilm positive. PCR evaluation results for the detection of clinical isolates blaTEM, blaSHV, blaCTX-M, intI1, fimH gene region; Of 120 isolates, 10 (8.3%) were blaTEM, 42 (35%) blaSHV, 23 (19.1%) blaCTX-M, 70 (58.3%) intI1, 27 (22.5%) is fimH positive. In conclusion, It is important to know the antibiotic resistance profile for the treatment of infections caused by Klebsiella, and by conducting studies to prevent biofilm formation, infections caused by K. pneumoniae will be prevented.
