The effect of chronic care model-based education and telephone monitoring on self-efficacy and patient-reported outcomes in COPD patients

Abstract (EN)
This research; It was conducted to evaluate the effects of education based on the Chronic Care Model given to COPD patients and telephone follow-up on the self-efficacy status of individuals and patient-reported outcomes. The research was conducted in two stages. As the first stage methodological study; It was conducted with 273 patients between September 2021 and January 2022. Modified Patient-Reported Outcome Scale for Chronic Obstructive Pulmonary Disease (mCOPD-PRO) was adapted to Turkish language and culture and examined its validity and reliability. As a second stage randomized controlled trial; It was carried out in Kahramanmaraş Provincial Health Directorate Elbistan State Hospital Chest Diseases Polyclinic with 61 patients between January 2022 and July 2022. In the first stage of the research; Content Validity Index of the scale was calculated as 0.91. It was seen that the DFA model fit index values for construct validity were within the desired range. The scale's Cronbach's Alpha reliability coefficient; It was found to be 0.954 in the mCOPD-PRO, 0.945 in the physical domain sub-dimension, 0.862 in the psychological domain sub-dimension, and 0.828 in the environmental sub-dimension. In the second stage of the research; training and telephone follow-up based on the Chronic Care Model in COPD were evaluated. Patients in the intervention group; It was determined that there was a significant increase in COPD Self-Efficacy Scale scores. Moreover; It was determined that the COPD knowledge level of the patients in the intervention group increased and their awareness of the disease increased. It was determined that the patients had a decrease in the mMRC dyspnea scale, and their dyspnea decreased. It was determined that there was a decrease in CAT scores, and a decrease in the scores of mCOPD-PRO and physical domain sub-dimension, psychological domain sub-dimension, and environmental domain sub-dimension. Accordingly, positive effects were found in the results reported by the patient. It was observed that the satisfaction of the patients with the chronic care they received was at a high level. In conclusion; The scale, consisting of 27 items and 3 sub-dimensions, was found to be a valid and reliable scale for Turkish language and culture. In COPD patients, education based on the Chronic Care Model and telephone follow-up increased patients' self-efficacy levels, had a positive effect on patient-reported outcomes, and increased their satisfaction with health care services. For this reason; Chronic Care Model-based training and telephone monitoring are recommended to be used in practice.
