Comparison of the effect of preoperative hemoglobin, albumin, lymphocyte, and platelet (HALP) score with cancer inflammation prognostic index (CIPI) on 5-year survival in patients with colorectal cancer

Aim: We aimed to investigate the prognostic role of preoperative HALP score and CIPI in patients undergoing surgery for colorectal cancer, and to compare their superiority to each other. Methods: Retrospective analysis was conducted from a prospectively collected dataset of 264 patients operated for colorectal cancer between June 2014 and March 2019. Statistical analysis of our study was performed using IBM SPSS Statistics 22 software. The association of HALP and CIPI indices with mortality was evaluated and a cut-off point was determined. According to this cut-off value, patients were divided into high and low groups for CIPI and HALP indices. Demographic, clinical, pathology data, and survival times were compared between groups. A p-value <0.05 was considered statistically significant in all statistical tests. Results: The cut-off value in HALP score was calculated as 28.43. Patients were divided into two groups based on HALP cut-off of 28.43 as low and high HALP score. Statistically significant differences were found between groups in terms of CA 19-9 levels, neoadjuvant therapy, stoma creation rates, intraoperative complication rates, pathological stage distribution, treatment response in patients receiving neoadjuvant therapy for rectal cancer, and total survival time. Patients were divided into two groups based on CIPI cut-off of 16.12 as low and high CIPI score. Statistically significant differences were found between groups in terms of CEA levels, CA 19-9 levels, elective case rates, conversion rates, pathological stage distribution, distant organ metastasis, and total survival time. Conclusion: In our study, we identified an association of low HALP and high CIPI scores with poor prognosis and poor clinicopathological data. Multi-center prospective studies are needed to investigate the superiority of HALP and CIPI scores to confirm our findings. Keywords: HALP, CIPI, Colorectal Cancer, Prognosıs
