The comparison of two new histologic grading of colorectal carcinoma with prognostic factors and survival

Colorectal cancer is the most common cancer of the gastrointestinal tract. When diagnosed at early stage, with minimal morbidity and mortality. This malignancy can be appropriately treated by curative surgery. Grading is an important prognostic factor in colorectal carcinoma. In this study 164 cases were selected, from January 2002 to April 2012 of Gaziantep University Faculty of Medicine Sahinbey Research and Training Hospital, Department of Pathology colorectal carcinoma due colectomy applied. All of H&E stained slides of the cases were revised and histological grade, lymphovascular invasion, perineural invasion, lymph node involvement , tumor size, stage, metastasis, recurrence were evaluated. In addition, based on the poorly differentiated component, two new histological grading were applied and correlated with survival rate. The prognostic factors and grading systems were compared with survival. To the statistical study of POR grading system with tumor size, tumor differentiation, tumor invasion, lymphovascular invasion, perineural invasion, lymph node involvement, metastasis and survival of relationships were significant. PDC grading system had a significant positive correlation with gender, tumor size, tumor differentiation, perineural invasion, lymph node involvement, metastasis, and survival. This grading system based on the poorly differentiated component differs radically from previously proposed grading systems to be superior in terms of pathological practice. It can help in the development of individualized postoperative treatment programs. In addition, this study was thought to be useful in determining of prognosis and overall survival of patients with colorectal carcinoma.
