The results of treatment and hydrophilic acrylic implantation after cataract surgery BY phacoemulsification in dogs

Abstract (EN)
In this study, it was subjected phacoemulsification methods and intraocular lens (IOL) implantation on a totally 19 dogs at different breed, age, sex and body weight. According to history and clinical examination, the cases were ranged as senile (10 cases), traumatic (8 cases) and metabolic (1 cases) cataract as etiologically. Five cases were hyper mature and 14 cases mature. Fifteen cases were treated by phacoemulsification and four cases by extra capsular cataract extraction methods. Twelve left and three right eyes with cataract were operated by phacoemulsification. Three left and one right eye with cataract were operated byextra capsular cataract extraction methods. Thirteen cases were implanted IOLafter phacoemulsification. One case was implanted IOL after extra capsular cataract extraction. IOL could not implant in two cases applied phacoemulsification and three cases applied extra capsular cataract extraction. Before operation, corticosteroid (10 mg/ml, prednisolone sodium phosphate, Norsol® - Mefar Pharmaceutical Ins.) was applied as 2 drops per day for one week. And then, a broad spectrum antibiotic (0.3% ofloxacin, Exocin® - Abdi İbrahim Pharmaceutical Ins.) was applied to eliminate seconder infection as 5 drops per day for a weekon both eyes. Topical tropicamide (Tropamid ® - Mefar Pharmaceutical Ins.), fenilefrin HCl (2.5% Mydfrin® - Alcon Laboratories) andcyclopentolate HCl (1%, Sikloplejin® - Abdi İbrahim Pharmaceutical Ins.) applied as 2 drops 2 times by interval 10 minutes, before one hour surgery, respectively. The anterior chamber was entered with two side-port on 3 and 9 position of o'clock by a blade (diameter 1.1mm). Anterior chamber was filled with trypan blue the anterior crystalline capsule. Ten or thirteen seconds after of the dye and its contact with anterior capsule of lens, the remaining dye was removed by washing the anterior chamber with balanced solution. The anterior chamber was then supplied with a viscoelastic substance of sodium hyaluronate (Bio-Hyolur SV® - BioTech 1-1.5ml). A main port incision (2.8 mm length) was made at 12 position ofo'clock for capsulorhexis and phacoemulsification procedures. After entering anterior chamber, it was formed a torn on anterior lens capsule by cystotome. A 360 degrees straight circular gap was created on centered of anterior capsule by holding side of torn by utrata forceps. Iris hooks were placed to provide pupil dilatation to two side ports and 1, 4 and 5 position of o'clock in cases with weakness of zonula and fall short of pupil dilatation. Acrylic hydrophilic monomer optic IOL of 41 D was placed into the capsule through special injector. Viscoelastic material was removed by irrigation and aspiration procedure after placing IOL. A 0.5 ml ofloxacin (Exocin® - Abdi İbrahim Pharmaceutical Ins.) and 0.5 ml prednisolone (Onadron® - İbrahim Etem Ulagay Pharmaceutical Ins.) against infection and inflammation into the anterior chamber after placing IOL was injected. A balanced solution was injected incision line (main and side port), thus, ıt was tried to supply closing incision lines by created edema. This procedure was sufficient for closing side ports. But this procedure was not sufficient in some cases, so corneal incision was closed with continuously absorbable 7/0 polyglactin 910 (Vicryl®) sutures. A broad spectrum antibiotic (ciprofloxacin, Ciloxan® - Alcon Laboratories) and corticosteroid (dexamethasone, Maxidex® - Alcon Laboratories) 8 times per day during first week against infection and inflammation was used after surgery on cases. The pomades were applied as 6, 5, 4, 3, 2, 1 times per day second, third, fourth, fifth, sixth, seventh week by decreasing, respectively. In addition topical antibiotic and corticosteroid, topical tropicamide (Tropamid ® - Mefar Pharmaceutical Ins.) 1 drop per day for 1 month was used for continuously mydriasis to prevent adhesions. Physiologic saline 5% was used to prevent potential edema development as 5 drops per day for a week. According to postoperative history and clinical examination of dogs, 14 cases were exactly healed, but it was not encountered with healing signs of vision due to postoperative complications in 5 cases.
