The relationship between severity of coronary artery disease with anxiety-depression

Background: Cardiovascular diseases are the leading causes of death around the world and the coronary artery disease (CAD) is one of the most common diseases in this category. Most previous research suggested that anxiety and depression have prognostic importance in patients with stable coronary artery disease (SCAD) and are associated with the development of coronary heart disease in initially healthy people. The purpose of this study is to show the anxiety and depression levels in patient who undergoing coronary angiography (CAG) with the initial diagnosis of Stable Angina Pectoris (SAP) and the relationship between severity of coronary artery disease with anxiety-depression. Material and methods: 138 patients admitted Dicle University School of Medicine Department of Cardiology between February and April 2015 and planned CAG to investigate SCAD and 31 healty adults selected among people who attended for routine medical checkup and medical examination of employment were included in this study. All patients were completed the questionnaires including State-Trait Anxiety Inventory-State, (STAI-S or STAI-1), Beck Depression Inventory (BDI) and Beck Anxiety Inventory (BAI) before at least 2 hours and at the same day coronary angiography. They recompleted STAI-S at the same day and at least 1 h later after the patients were informed about their coronary artery lesions. Healty adults who were in control group completed only BAI and BDI. The severity of CAD disease was assessed using Gensini score. Results: BDI scores of patients were classified as normal coronary artery (NCA), noncritical and critical coronary artery stenosis were 12.5, 13.5 and 11.8(p>0.05), BAI scores were 42.7, 43.2 and 42.5 (p>0.05). BDI, BAI scores of the control group that consisted of healthy adults were 17.8 and 16.2 (p<0.05). BDI scores were 12.6, 12.2 in low Gensini score group and high Gensini score group (p>0.05). BAI scores were 17.8, 16.2 in low Gensini score group and high Gensini score group (p>0.05). Conclusions: Among patients who admitted chest pain, underwent coronary angiography for suspected SCAD and esstablished NCA, noncritical coronary artery stenosis and critical coronary artery stenosis, there was no statistical significant difference between those groups for anxiety and depression scores. But all groups had significantly higher anxiety and depression scores as compared with the anxiety and depression scores in control group included healty persons. Among patients established CAD, there was no correlation between Gensini score and anxiety, depression scores. Keywords: Coronary Artery Disease, Anxiety, Depression
