The association of coronary artery disease with neutrophil gelatinase?associated lipocalin (NGAL), adiponectin and pregnancy associated plasma protein A (PAPP-A) levels

Aim: The severity and extent of coronary artery disease can be examined by the Gensini score. The aim of our study is to assess NGAL, Adiponectin, PAPP-A, IGF-1 and IGFBP-3 levels in four group of patients; Group 1(n=39) 1 vessel disease, Group 2 (n=40) 2 vessel disease, Group 3, (n=39) 3 vessel disease and the control group without stenosis (n=40). The purpose of the present study is to investigate the association of the above markers and the number of vessels with stenosis, the Gensini scoring and the usability of the parameters as early markers.Methods: All patients were collected from the cardiology outpatient clinic with Coronary Artery Disease symptoms. Coronary angiography was planned for all 158 symptomatic patients with atherosclerosis. All patients had coronary anjiograpy and Gensini scoring for the assessment of severity of atherosclerosis. Peripheral venous blood TG, HDL, LDL, T. Kol., Urea, creatinine, Potassium, IGF-1, IGFBP-3, PAPP-A, NGAL, Adiponectin levels were assessed from all patients and the control group before the angiography. SPSS 10.0 had been used for statistical analysis.Results: Gensini score of the control group, Group 1, Group 2 and Group 3 were 0.0±0.0, 10.29±10.79, 23.92±21.79 and 56.51±35.39 respectively. The mean PAPP-A, NGAL and Adiponectin levels were statistically different among the groups (p=0.001, p=0.035, p=0.013, respectively). The mean PAPP-A levels in group 1 is statistically different from the control group (p=0.001). The mean NGAL levels in group 2 is statistically different from the control group (p=0.018) and the mean adiponectin levels in group 1 (p=0.014) and 2 (p=0.048) were the statistically different the control group. IGF-1 and IGFBP-3 levels were not statistically different in any of the groups (p>0.05).Conclusions: The togetherness of silent plaque and ruptured or eroded plaques in the coronary artery disease group influenced the levels of our parameters. We did not find any correlations with the number of diseased vessel, Gensini score and the assessed parameter levels. However because of the significance in group one and two of adiponectin levels we conclude that adiponectin can be a diagnostic parameter in a larger group of patients with different number of vessels involved. PAPP-A can be a diagnostic tool in patients with one vessel disease.Keywords: Coronary artery Disease, Coronary Angiography, Gensini Score, Adiponectin, PAPP-A, NGAL
