Comparison of multislice computed tomography and invasıve coronary anjiography in evaluation of coronary artery pathologies

Abstract (EN)
Diagnosis of the coronary artery disease has great importance because of being an epidemic disease. Invasive coronary angiography is a method which is accepted as a gold standard in this subject. The technologic developments in scanners showed that multislice computed tomography (MSCT) coronary angiography can determine pathologies in coronary arteries sensitively for the patients with low heartbeat, whose MSCT coroner angiography were obtained by using suitable procedures.The aim of this study is to reveal advantages and disadvantages of MSCT coronary angiography in comparison with invasive coronary angiography.Fifty patients who had invasive coronary angiography and directed to the department of Radiology at Gaziantep University Hospital were enrolled in the study. Eighty-four percentage (n = 42) of patients were male and 16% (n = 8) were female. Mean age of the patients was 56 ± 4 years and mean heart rate was 59 ± 5 beats per minute during the shooting. All patients? coronary arteries were classified according to the American Heart Association (AHA) classification as proximal, middle and distal segments. Our results were compared with those of invasive coronary angiography.In our study, the sensitivity of MSCT coronary angiography was 95%-92%, the specificity was 97%-96%, PPV was 94%-80%, and NPV was 97%-99% in middle and distal segment stenosis, respectively. In determination of stenosis of proximal and middle 135 segments, which were occluded ?50%, the sensitivity of the method was 97%, the specificity was 99%, PPV was 97%, and NPV was 99%. Our success in determination of stenosis in proximal and middle segments was higher than distal segments. In proximal-middle segments, which had medium-high stenosis degrees, MSCT angiography detected more successfully than minimal stenosis. In ostial lesions the reliability of MSCT angiography was found higher.As result, MSCT coronary angiography can be done during a single breath-hold, has advantages such as patient comfort and rare potential complications. It can be used in symptomatic or asymptomatic coronary artery disease in patients with low-intermediate risk for screening and diagnostic purposes as a non-invasive imaging technique.
